Surgical management of recurrent shoulder instability can be complicated in the setting of associated osseous defects of the glenoid, humeral head, or both. A wide variety of surgical options exist for the management of complex shoulder instability. Interventions for addressing glenoid and humeral head bone defects, and their biomechanical effects, are reviewed. Further studies are required to delineate critical defect values and develop validated treatment algorithms. © 2013 Elsevier Inc.
