Abstract
The CI-6 yields a standardized interment for clinical and research purposes to assess the treatment compliance of in-patients with schizophrenic and schizoaffective disorders retrospectively. Global treatment compliance was defined as voluntary and insightful participation in medical and psychosocial treatment recommendations. Method: The CI-6 consists of 6 items concerning insight, voluntariness, and participation in medical and psychosocial treatment recommendations. Interrater reliability, external validity and empirical overlap among insight, voluntariness, and treatment participation were examined in a sample of n=52 patients suffering from schizophrenic or schizoaffective disorders. Results: The interrater reliability yielded good agreement on all 6 items (each kappa > 0.75). Better treatment compliance correlated significantly with older age, female gender, schizoaffective disorder, voluntary admission, and clinical global impression at discharge. There was a substantial but in no way complete overlap among insight, voluntariness, and treatment participation. Particularly voluntariness without insight was observed relatively frequently. Conclusion: Our first reliability tests provided evidence that the CI-6 is a reliable instrument for the retrospective assessment of treatment compliance in in-patients with schizophrenic disorders. At least, it should be considered for many clinical and research purposes, whether the CI-6 should be preferred to the merely dichotomous categorization in compliant and noncompliant medication in-takers.
