Young male, height-175 cm, no comorbidity presents with rapidly worsening respiratory failure. No other organ involvement. On controlled mechanical ventilation. Vt 420 mL, FiO2-1.0, PEEP 18 cm H2O, Pplat 34 cm H2O (under paralysis). ABG analysis shows: PO2-45 mm Hg, PCO2 89 mm Hg, pH 7.14, HCO3-28. How do we proceed?.
