Endnote 6 should read U.S. Nuclear Regulatory Commission rather than National Research Council. The date of the reference by Fischhoff et al. should be 1977. The last line of the caption of Fig. 17 should read: Results developed by Fischhoff et al. (1977) and originally plotted by Vick (1997, 2002). The following reference should be added: Vick, S. G.
