Abstract

COVID-19 is confronting health systems with an enormous demand for services. A large part is the vulnerable urban population due to age, comorbidities, and socioeconomic status. We linked two methods focused on achieving a more accessible, efficient, and equitable socio-spatial distribution of the health system for older adults, in an urban and dense municipality of Mexico, with general conditions of poverty. The first method measures the levels of accessibility of the elderly population to health units, with walking transfers (called the Two-step floating catchment area); the second assesses the most convenient locations for new units, combining criteria of efficiency and equity (the p-Median model with distance restrictions). We revealed inequalities of accessibility of older adults to health units and the most satisfactory locational solution, assuming an increase of six new health units. It is proposed to streamline the used methods with Cellular Automata.El COVID-19 enfrenta a los sistemas de salud a una enorme demanda de servicios. Gran parte es población urbana vulnerable por edad, comorbilidades y condición socioeconómica. Aquí se vinculan dos métodos enfocados a lograr una distribución socioespacial del sistema de salud más accesible, eficiente y equitativa para los adultos mayores, en un municipio urbano y denso de México, con condiciones generales de pobreza. El primer método mide los niveles de accesibilidad de la población mayor a las unidades de salud, con traslados caminando (llamado Área de cobertura flotante de dos pasos); el segundo, valora las localizaciones más convenientes de nuevas unidades, combinando criterios de eficiencia y equidad (el Modelo p-mediana con restricción de distancia). Se develan desigualdades de accesibilidad de los adultos mayores a unidades de salud y la solución locacional más satisfactoria, proponiendo un incremento de seis nuevas unidades. Se plantea dinamizar los métodos utilizados con Autómatas Celulares.
