Abstract

Assessing and prioritizing risks in the chemotherapy ordering and prescribing processes is crucial to improving their safety and quality. While FMEA is commonly used for this purpose, it has some limitations. To overcome these limitations, a three-stage approach was proposed in this study to enhance the FMEA method. The first stage involved using FMEA to identify and assign values to RPN parameters. In the second stage, the fuzzy SWARA method and expert opinions were used to calculate the weights of the three factors. Finally, in the third stage, the risks were prioritized using the Z-MOORA approach, which provides more accurate results due to its consideration of different factor weights, uncertainty, and the use of the Z-number theory for reliability.
