Measurement of N-terminal pro-B-type natriuretic peptide(NT-proBNP)in hemodialysis patients-Examination by electrocardiogram-

Abstract

Key words：hemodialysis patients, N-terminal pro-B-type natriuretic peptide（NT-proBNP), electrocardiogram, prognostic factor, heart failure 〈Abstract〉 N-terminal pro-B-type natriuretic peptide（NT-proBNP）is a new biomarker of both the cardiac function and heart failure. The serum concentrations of NT-proBNP increase in hemodialysis（HD）patients. This study examined the usefulness of NT-proBNP measurement in HD patients by examining the relationship between NT-proBNP and electrocardiographic（ECG）findings. This study observed 85 HD patients. The serum concentrations of NT-proBNP were measured by an electrochemiluminescence immunoassay（Elecsys 2010, Roche Diagnosics). RV5＋SV1 voltage, RV5 voltage, QRS interval, QTc interval and QRS axis were used as ECG ／ parameters. The serum NT-proBNP concentrations in HD patients（676-127,172 pg／mL）were significantly ／ higher than healthy subjects（9-144 pg／mL). There was a significant positive correlation between NT-proBNP ／ and RV5＋SV1 voltage. A receiver-operating characteristic curve demonstrated about 8,000 pg／mL as the cutoff 蔦谷 知佳子 鷹揚郷腎研究所 〒 036-8243 青森県弘前市小沢字山崎 90 Chikako Tsutaya Tel：0172-87-1221 Fax：0172-87-1228 〔受付日：2010 年 2 月 16 日，受理日：2010 年 5 月 11 日〕 ヒト脳性ナトリウム利尿ペプチド前駆体 N 端フラグメント（NT-proBNP）は一般住民において心機能マーカー として有用とされている．しかし，NT-proBNP は，主に腎臓で代謝されるため，血液透析（HD）患者では高値を 示し，その基準値は必ずしも明らかでない．著者らは HD 患者における NT-proBNP 濃度の基準値を求め，心不全 の予後予測因子としての有用性について検討した．対象は HD 患者 85 例である．血清中の NT-proBNP 濃度は電 気化学発光法（エクルーシス proBNP，ロシュ・ダイアグノスティックス）で測定し，心電図からは V5 の R 波振幅 ＋V1 の S 波振幅（RV5＋SV1） ，RV5，QRS 間隔，QTc 間隔および QRS 軸を読んだ．HD 患者の NT-proBNP 濃度 ／ ／ （676-127,172 pg／mL）は健常人（9-144 pg／mL）にくらべ有意に高値であった．NT-proBNP は心電図上の左室 肥大診断に最もよく用いられる RV5＋SV1 と r＝0.346（p＝0.002）の正相関を示した．RV5＋SV1B3.8mV を左室 ／ 肥大とし，診断特性（ROC）曲線から心機能異常を推定できうる NT-proBNP の基準値を約 8,000 pg／mL とした． ／ この NT-proBNP 8,000 pg／mL を基準に HD 患者を 2 群に分け，採血後から 1 年間の心不全発生率を比較した． ／ ／ NT-proBNPB8,000 pg／mL 群（n＝35）の心不全発生率は 31.4％であり，NT-proBNP＜8,000 pg／mL 群（n＝50） の心不全発生率 0.0％にくらべて有意に高かった（p＜0.0001） ．以上より，NT-proBNP は HD 患者においても心不 ／ 全の予後予測因子として有用であり，その基準値は 8,000 pg／mL が適当と考えられた． 〈要旨〉 キーワード： 蔦谷 知佳子 對 馬 惠 寺山 百合子 山 谷 金 光 齋 藤 久 夫 舟 生 富 寿 鷹揚郷腎研究所
