Abstract

Acute scrotum is defined as the development of testicular pain within a short period of time, usually with scrotal swelling signs. Testicular appendix torsion, epididymitis-orchitis and testicular torsion, are the three main causes of this entity, which in most patients are diagnosed by history taking and scrotal examination. Acute scrotum is considered a medical emergency that requires an early diagnosis in order to urgently treat the cases of testicular torsion. In the latter, the surgical procedure consists of testicular detorsion and orchidopexy, provided confirmation of testicular viability. Should testicular necrosis be observed in the surgical exploration, orchiectomy is mandatory. Medical treatment with rest and anti-inflammatory drugs is the treatment of choice in cases of testicular appendix torsion and epididymitis-orchiepididymitis. In patients with epididymitis and urine or urethral infection, antibiotic treatment is also required.
