Hypertension is a common disease especially among the elderly, which is an abnormal increase in blood pressure in the arteries continuously with systole more than 140 mmHg and diastole more than 90 mmHg. Hypertension and aging if not treated immediately and left for long time can have an impact on kidney function, kidney failure, coronary heart disease and stroke. Currently the morbidity rate of hypertension in elderly still high, there are many factors that influence, especially medication adherence factor. A study determine factors associated with adherence to hypertension treatment in elderly. This study uses literature review and database design to search for literature reviews using keywords ((((Hypertension) and Aged) or elderly) AND Medication Compliance) or adherence, medication) and factors). 5 articles were selected using JBI Critical Appraisal tools. Articles were taken from reputable and accredited database, namely 4 articles from PubMed database, while an article was accredited from Google Schoolar. The selection was taken by paying attention to PEOS, then analyzed one by one. The results factors related are include age, gender, education level, physical status (weakness) and mental (cognitive function). There are two factors that are strongly related: namely family support and social support. Compliance with hypertension treatment in elderly needs to be improved, so elderly need social support and family support to undergo hypertension treatment compliance along with changes associated with aging process. Introducing community and families to always provide support for elderly to be obedient in treatment
