Abstract

Background: Our previous observation suggests that poor sleep is a possible cause of ischemic heart disease (IHD) through its adverse effects on blood pressure level, circadian rhythm, insulin sensitivity, glucose metabolism, sympathetic nervous system and hypothalamic-pituitary-adrenal axis. However, the hypothesis has not been tested whether poor sleep could directly deteriorate cardiac function in elderly people. Purpose: We investigated the association between sleep quality and serum Nterminal pro-brain natriuretic peptide (NT-proBNP) level, a marker for ventricular dysfunction, in examinees of annual health check. Methods: This is a cross-sectional study involving 2,358 participants without IHD, aged 68 years or older, who underwent general health examinations. We assessed serum NT-proBNP level and defined high NT-proBNP level as ≥125 pg/ml. Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI), which yields seven component scores (0, 1, 2, or 3 is given for each component). Based on the PSQI component scores, we defined subjective poor sleep quality, difficulty in initiating and/or maintaining sleep (sleeping difficulty), short sleep duration, use of sleeping medications, and daytime dysfunction as listed in the Table. Results: The median (25%-75% interquartile range) NT-proBNP level was 79.5 pg/ml (range, 47.0-144.0 pg/ml), and 717 participants (30%) had high NT-proBNP level. High NT-proBNP level was significantly associated with sleeping difficulty, and short sleep duration, but not subjective poor sleep quality, use of sleeping medications, and daytime dysfunction (Table). All the data were adjusted for age, gender, body mass index, hemoglobin concentration, serum creatinine, and presence of hypertension, atrial fibrillation, and other cardiac diseases including valvular heart disease and cardiomyopathy. (Table Presented) Conclusion: Our results suggest that sleeping difficulty and short sleep duration are associated with ventricular dysfunction in elderly people.
