A 58-year-old gentleman admitted to the intensive care unit (ICU) with Pneumocystis jirovecii pneumonia and advanced HIV developed a potentially lethal hyperkalaemia secondary to his antibiotic treatment. The mechanism and treatment are described. © The Intensive Care Society 2013.
