Abstract

ABSTRACT We estimated the costs of a molecular test for Mycobacterium tuberculosis and resistance to rifampin (Xpert MTB/RIF) and of smear microscopy, within the Brazilian Sistema Único de Saúde (SUS, Unified Health Care System). In SUS laboratories in the cities of Rio de Janeiro and Manaus, we performed activity-based costing and micro-costing. The mean unit costs for Xpert MTB/RIF and smear microscopy were R$35.57 and R$14.16, respectively. The major cost drivers for Xpert MTB/RIF and smear microscopy were consumables/reagents and staff, respectively. These results might facilitate future cost-effectiveness studies and inform the decision-making process regarding the expansion of Xpert MTB/RIF use in Brazil.RESUMO Foram estimados os custos unitários de um teste molecular para Mycobacterium tuberculosis e resistência à rifampicina (Xpert MTB/RIF) e da baciloscopia sob a perspectiva do Sistema Único de Saúde (SUS). Foi utilizado o método de custeio por atividade com a técnica de microcusto em laboratórios do SUS nas cidades do Rio de Janeiro e Manaus. As médias de custo unitário foram de R$ 35,57 e R$ 14,16 para Xpert MTB/RIF e baciloscopia, respectivamente. Os principais direcionadores de custo do Xpert MTB/RIF e da baciloscopia foram, respectivamente, insumos/reagentes e recursos humanos. Estes resultados podem contribuir com estudos futuros de custo-efetividade de novos testes e com o processo de tomada de decisão acerca da expansão da adoção do Xpert MTB/RIF no Brasil.
