An exquisite perioperative management, besides an adequate surgical technique, is mandatory to obtain good outcomes in coronary artery bypass grafting surgery (CABG). So, all the steps related to the surgical procedure should be optimized, from the beginning of the process, when surgical candidates are selected for CABG, to the recommendations and medications prescribed at Hospital discharge. There are several steps that can be modified to improve the early and late surgical results. In this review we will deal with some of this factors, such as preoperative risk estimation taking into account the frailty of the patient, ischemic preconditioning, blood transfusions, intensive care unit management and fast track protocols.
