Acute Lung Injury (ALI) and its severe form Acute Respiratory Distress Syndrome (ARDS) are clinical syndrome characterised by impairment in gas exchange and/or lung mechanics that leads to hypoxemia and increased work of breathing (WOB). The common causes are aspiration pneumonia, endotoxin exposure (lipopolysaccharide), inhalation injury, fat emboli, or indirect insults such as Sepsis, severe trauma and drug overdose. Besides, genetic factors also contribute to susceptibility and prognosis of ALI/ARDS. Shortness of breath, fast breathing, and a low oxygen level in the blood are the symptoms. ALI diagnosed by bilateral opacities on chest imaging (chest radiograph or CT). The mortality rate for ARDS is between 36% and 44%. ALI/ARDS are characterised by severe inflammation within the lung leading to disruption of alveolar endothelial barrier, which results in an influx of excessive neutrophils and other inflammatory cells, a release of inflammatory and cytotoxic mediators, alveolar flooding with protein-rich fluid, impaired surfactant synthesis and metabolism, and a local pro-coagulant state. Till now, there is no effective treatment available for ALI/ARDS. Taking into account that MicroRNAs (miRNAs) are endogenous single strand, non- coding RNAs of 18-25 nucleotides in length, MiRNAs primarily bind to the 3'-untranslated regions (UTR) of the mRNAs to inhibit protein translation or cause mRNA degradation. miRNA plays the critical role in the various physiological and pathological events of lung injury
