Type I endoleaks occasionally persist after endovascular repair of thoracic aneurysms. A technique for repair is described using dimensions obtained from preoperative imaging, in which the aorta is banded at the endograft attachment zone. Aortic remodeling and aortic dimensional stability are the mainstays of the approach. Measured banding has also proved to be useful for the preparation of a landing zone prior to endograft placement during hybrid procedures. © 2008 The Society of Thoracic Surgeons.
