We are constantly reminded of the high numbers of incidents or occasions of patient safety incidents, never events, deaths, hospital acquired infections and complaints that fill our journals, newspapers and media, in relation to the care we strive to deliver to our patients. I know that this is becoming a real source of anxiety as I speak to colleagues and members. The distress and frustration is almost tangible as they work under the enormous pressures of today's healthcare environments.
