A man aged 40 with a large sphenoid and ethmoid sinus pyocele developed headaches, vomiting, and visual disturbance. Radiological studies showed the destructive finding of the sphenoid sinus and opacification of the ethmoid and sphenoid sinuses. The lesion produced displacement and stenosis of the internal carotid artery on the arteriogram. The outline of the lesion was clearly shown in the preoperative CT-scan. The symptoms were perfectly improved after surgery by extranasal approach to the lesion.
