The authors report a case of fracture of the os peroneum with diastasis in a 34-year-old woman. This represents one of the few cases of this type of fracture to be reported in the literature. This injury must be included in the differential diagnosis of a sprained ankle, and only careful examination of serial roentgenograms can prevent an incorrect diagnosis. © 1995, American College of Foot and Ankle Surgeons.
