This article shows development of therapeutic design for a 14-year-old fema- le adolescent with self-injurious behavior as an emotional regulation tool in different self states. The objective is to try and show the importance of not limiting our attention to the symptom of self-harm, however impacting it may be, but investigating the complexity of its origins and functions, and to make clear the importance of therapeutic action from a re- lational perspective, focusing attention on the complexity of what occurs at every moment of encounter and disagreement. The importance of emotional regulation throughout life, for mental health, and in the psychotherapeutic context is also stressed. Before presenting the case, a theoretical introduction will help understand this work
