Abstract

INTRODUCTION AND AIMS: The growing development of living donor kidney transplant (LDTX) has led to an extension of donor selection criteria. During the evaluation of the potential candidate, subclinical diseases are often first diagnosed, with consequent improvement of life expectancy. Aim of this study is to analyze the frequency of new diagnosis in potential kidney donors. METHODS: We performed an observational retrospective study: all 76 potential kidney donors evaluated in Our Transplantation Unit from 01/01/2014 to 10/06/2016 were included RESULTS: The following diseasewere diagnosed during evaluation of candidates. Cardiovascular disorders: hypertension in 6/76 patients, arterial aneurysms in 4/76 and stenosis in 1/76;metabolic disorders: diabetes mellitus type 2 in 1/76, metabolic syndrome in 1/76, significant overweight with dietological indication in 14/76; haematological disorders: hereditary or acquired thrombophilia in 8/76; pneumological disorders: lung nodules that required follow-up 4/76, quantiferon TB positivity 6/76. Endocrine disorders: chronic autoimmune hypothyroidism8/76, thyroid nodules which underwent fine-needle aspiration biopsy 5/76, adrenal incidentalomas 2/76.Helicobacter Pylori gastritis in 4/76, pre-cancer conditions (gastric intestinal metaplasia, low-grade dysplasia colon and uterine cervix polyps) in 8/76, 2/76 active malignant neoplasms.Nephro-urologic pathologies (CKDstage II, malformative uropathies) 7/76. Infections: 1/76 Ascariasis. CONCLUSIONS: Work-up to assess eligibility of kidney donors often led to diagnosis of subclinical disorders and consequently to specific follow-up or therapies, with an improvement in life expectancy of candidates to donation. In most cases donors were considered eligible (36/76, 47.4%), whereas only 11/76 (14.4%) had absolute contraindications; in 29/76 the evaluation was interrupted early (recipient non eligible for transplant, positive cross-match, lack of motivation) so that the study had not been completed. At least 4 donors improved their life expectancy (early diagnosis of cancer) by proposing themselves as potential donors for LDKT.
