Soon after the introduction of the Kock pouch it became obvious that different complications threatened the pouch function. Most of the late complications affect the nipple valve, leading to difficulties intubating the pouch or leakage. In 1976 Kock described some of the procedures for revisional surgery used today. Patients who have experienced a functioning continent ileostomy are ready to go through revisional surgery more than once to save the pouch. How to surgically manage early and especially late complications, will be described in the following chapter.
