A 68-year-old man was seen at the preoperative evaluation clinic 1 month prior to transurethral resection of bladder tumor. He had a past history of anterior myocardial infarct, hypertension, and diabetes mellitus (type 2). Six weeks prior to this anesthesia consultation, the patient had a 2-week admission for treatment of an acute exacerbation of congestive cardiac failure. He was noted to belong to New York Heart Association (NYHA) Functional Classification III during this hospitalization period. This had not changed in the intervening 6-week period. He has been compliant with prescribed medications since discharge.
