The helical crus normally separates the concha cavum and the concha cymba. Absence of the helical crus may contribute to the appearance of the deep conchal bowl. A technique to create the helical crus and thereby improve the appearance of the conchal bowl is presented. Shaving of eminentia on the posterior aspect of the conchal cartilage may also be a helpful adjunct in cosmetic otoplasty. Accentuation of the helical crus and shaving of the eminentia can be used to create a more normal appearing ear while minimizing the complications associated with other techniques.
