Milestones in the history of the development of vascular access and the subsequent advances in practical clinical applications are here described. The original achievements are presented and discussed. Fundamental steps to achieve a safe and effective IV access and management are pointed out. Peripherally inserted central catheters (PICCs) are the newest devices; they afford clinicians a safe and effective option for central access that is required for a variety of therapies in the pediatric and adult population. The relatively low complication rates and ease of placement, particularly with newer methods and the addition of technological assistance of ultrasound guidance, have propelled PICCs as the predominant choice for central venous access and opened a new era in the field of intravenous therapy.
