Abstract

Survivorship is an integral step in your breast cancer patient's care continuum. Patients have voiced concerns that they are well taken care of during treatment but feel dropped "like hot potatoes" when it is completed. Cancer Survivorship Programs help bridge the gap from active treatment to the transition back to the primary care physician and life without treatment and the security of their cancer care team. The American College of Surgeons Commission on Cancer has added Standard 3.3 Survivorship Care Plan, addressing the need to help fill the gap and summarize information needed to care for cancer survivors in the long term.
