A 72-year-old man was admitted to the cardiology service for elective mechanical aortic valve revision. He had symptomatic, severe aortic regurgitation. For several months, he reported having gradually progressive palpitations, lightheadedness, orthopnea, dyspnea on exertion, and nonproductive cough. On the day of admission, he reported that during the previous 2 weeks, he had had drenching night sweats. He reported an unintentional weight loss of 22 lb over 8 months.
