Abstract

Successful management of chronic disease requires the patient to carry out treatments recommended by healthcare professionals. Patients must be well-informed collaborators in their own care. Using a shared decision-making process, health professionals help the patient make informed decisions that achieve agreed-upon goals by providing education, appropriate care recommendations, expert advice, and support. Self-management prepares patients to care for their health, keeping wellness as a focus. It involves techniques that enhance empowerment and self-efficacy in the setting of the office or home. Support comes from the provider, the community, and peers.
