Laparoscopic adrenalectomy for a large pheochromocytoma

Abstract

Laparoscopic adrenalectomy has been performed for pheochromocytoma scarcely and to date it has not been done for adrena1 masses greater than 6 cms. Th i s is the first case, where we have removed large size pheochromocytoma (measuring 7.2 × 5.5 × 5.1cm) laparoscopically by transperitoneal access. This video demonstrates point of technique of removing such a tumour. The operative time was 130 minutes with blood loss of 120CC. Patient was discharged on 3rd day. The role of triflanged retractor and precise dissection are evident in the operative procedure. A case of pheochromocytoma can be dealt laparoscopically after adequate preparation with pharmacological agents. However, it needs great precision and skill and equal, if not more, intraoperative anaesthetic care and monitoring as for open surgical excision. Post-orative course is smoother and with lesser pain and ileus.
