Abstract

In this video, we present D2 gastrectomy including exploration, vascular disposal, lymph nodes dissection and reconstruction. We very much focus on clear anatomic layer and precise operation. We apply the routine laparoscopic approach in open gastrectomy for lymphadenectomy. It is probably a safe and feasible approach to dissect the No. 12a and 8a lymph nodes. And it's efficient to avoid damaging the proper hepatic artery (PHA) and coronary vein. Furthermore, exposed in situ by anterior and posterior pancreatic approach, splenic hilum is reliably exposed and the No. 10 lymph nodes are easily and thoroughly dissected.
