Abstract

This research proposes a solution to establish an effective chronic disease prevention and control mechanism to strengthening public health management in China. This approach is to change from passive medication to active healthcare, which aims to prevent chronic diseases at early stage. The practice in developing one key subsystem of the proposed solution is demonstrated. It provides an intelligent medical platform that links hospitals, patients and community healthcare centers via the Internet to effectively achieve self-monitoring of health conditions and personalized medical treatment of hypertension.
