Maxillomandibular advancement surgery has been a mainstay for treatment of obstructive sleep apnea since the mid-1980s. Multiple and significant advances have been made since then, both regarding the surgical procedure and treatment options for obstructive sleep apnea. Maxillomandibular advancement, however, remains the most reliable and successful surgical treatment for severe obstructive sleep apnea, resulting in the largest postoperative decrease in AHI and PAP use. In this chapter, the history, surgical anatomy, surgical procedure, success outcomes, and complications of maxillomandibular advancement surgery are discussed in detail.
