Transposition of the great arteries (TGA) is a congenital malformation with discordant ventriculoarterial connections. The newborn circulation runs in-parallel as opposed to in-series, and cardiovascular collapse occurs without adequate mixing between these circulations. Detailed anatomic assessment of associated lesions and coronary arterial pattern are required for preoperative planning. Uniformly fatal without correction, modern surgical therapy with the arterial switch procedure has produced excellent outcomes with >90% survival. The chapter provides an overview of the epidemiology, physiology, perioperative imaging, medical and surgical management as well as outcomes of TGA.
