Abstract

Endoscopic surgery offers multiple advantages in the approach to lesions of the ovaries. Laparoscopic approach offers a more conclusive diagnosis as well as the option of treatment in benign ovarian pathologies. Simple ovarian cysts can be punctured and aspirated under visual guidance. However, in case of large cysts with torsion of the ovary, endoscopic manipulation can relieve the torsion as well as secure the ovarian tissue to the edge of the pelvic floor. Operation room setup, patient positioning, special instruments and port placement sites for the laparoscopic management of ovarian cysts have been outlined. The indications, contraindications, preoperative considerations, technical notes and procedural variations have been listed. High-quality images with captions demonstrate the treatment strategy. © 2009 Springer-Verlag Berlin Heidelberg.
