Abstract

This workshop is dedicated to learning from low level incidents, which taken together indicate areas for significant loss, if no appropriately acted on. These low level "precursors" in themselves do not result in any measurable detriment to the organization, unlike the significant incidents which normally are the ones given most attention and follow up. © 2006 Springer.
