ACL graft fixation implants can be associated with complications like divergence, soft tissue irritation, and tunnel widening. Press-fit anterior cruciate ligament (ACL) reconstruction is a novel concept, which was introduced by Peter Hertel in 1987. This idea appealed to other authors who extended its use to different graft types and all arthroscopic techniques. It has the additional advantage of diminished cost, quicker incorporation of graft, and ease of revision surgery. Biomechanical strength testing results have been encouraging, and various authors have presented good to excellent long-term clinical results. In this chapter, we will discuss the history, surgical techniques, biomechanical evidence, and clinical results of press-fit technique.
