Care of the patient on ECLS requires attention to the complex circuit and a complicated patient. A systematic, daily approach is essential for safety and depends on highly experienced, diligent staff (Butt et al., Pediatr Crit Care Med 14(5 Suppl 1):S13–19, 2013). This chapter presents a thorough approach to examination of the ECLS circuit, seeking to optimize its function and predict or prevent complications. In addition, we advocate a multisystem approach to the patient that considers issues of early stabilization, later maintenance, and, finally, weaning and decannulation. Overall care is often enhanced by melding protocols and approaches with those practiced in other critically ill patients managed in the same ICU.
