A greater understanding of midfacial anatomy and its contribution to the dynamics of facial aging has led to an increased interest in midfacial rejuvenation. Adequate midfacial ptosis correction usually cannot be achieved with percutaneous and injectable methods alone. Transeyelid and subperiosteal approaches to the midface can also lead to potential undesirable complications. The preperiosteal midface lift, accomplished through a transtemporal approach, is an excellent method for elevation of the midface in an anatomically safe midface plane. This procedure is usually performed in conjunction with endoscopic browlift. Results are aesthetically pleasing and associated with few postoperative complications. This chapter describes several surgical maneuvers that facilitate the procedure.
