Abstract

Error states exist as part of all technologies, and the challenge is therefore to eliminate them, not just partially correct them. This paper provides the seeds of a novel approach, by examples and theory, that provides a new way forward. We review some technological examples where improved Error State analysis coupled with training could have saved serious consequences. Ideally, Error States can be eliminated by dynamically tracking our progress as we continually learn and improve our ability to predict events, and by enforcing and/or requiring the most effective approaches to ensure maximum error state elimination.
