Acute redness, swelling, and pain of the scrotum are symptoms associated with a range of differential diagnoses. Of these, spermatic cord torsion is the most urgent diagnosis that requires immediate surgical intervention to attempt testis salvage. The challenge for the provider is to expeditiously synthesize the details of a history and physical examination to determine whether testis ischemia is likely and whether the case should proceed emergently to the operating room.
