Leptospirosis is a zoonosis of worldwide distribution. Its clinically more severe form is called Weil Syndrome, which is characterized by generating a severe and frequently fatal multiorgan systemic impact. Numerous wild and domestic animals constitute the reservoir and become asymptomatic carriers, being a source of infection towards humans. The diagnosis is based on clinical and epidemiological suspicion, confirming with serological studies. The treatment is symptomatic and with targeted antibiotics. Below is a case of leptospirosis that presents with acute appendicitis with severe organic involvement not consistent with the initial abdominal pathology.
