In contrast to concerned persons and to medical institutions, coronary artery disease (CAD) is the leading cause of mortality also in women, although the time of the first manifestation is about ten years later compared to male patients. The risk profile for CAD is comparable yet each risk factor is much more relevant compared to females who do not show these risk factors. Also diagnostic procedures to establish or exclude the diagnosis are less sensitive and specific for females. Mortality during acute myocardial infarction is also different, not only related to age and co-morbidity, but also related to the therapeutic approach that is used: whereas thrombolytic therapy is associated with a far higher mortality among women, females show a favourable outcome when PCI has been performed. The role of HRT with respect to primary and secondary prophylaxis is established with respect to the large studies available and has no general role.
