Abstract

The field of care coordination and professional designation of case manager is growing in response to evidence-based studies, increased numbers of vulnerable children and families needing support, models of care delivery featuring care coordination to ensure continuity and monitoring of needs, and due to outcomes requirements being put in place by regulatory and accrediting entities that include face-to-face coordination and support. Technology will assist in this industry explosion, coordinating care with alerts, reminders and communication enhancements. Whether innovative devices, shared case management platforms or online educational and support groups, technology is paving the way for greater efficiency and effectiveness of care coordination teams. The interprofessional activity required by care coordination will transform the culture of healthcare delivery resulting in a true family-centered focus of assessment, identification of barriers and support in goal attainment using a team approach.
