In this chapter, we will explore a case involving a young adult with a history of depression who attempts to end his life using a firearm. Questions will guide exploration of patient barriers to receiving mental health care, firearm-related violence statistics, and physician responsibilities with regard to maintaining patient confidentiality and obtaining informed consent. While working through the case, learners will be challenged to critically assess a clinical scenario and offer a framework for identifying potential interventions. At the conclusion of the case discussion, learners should be able to choose among potential courses of action when an obligation to maintain confidentiality conflicts with a duty to protect the patient and their community.
