Abstract

Transitional care becomes more and more essential for pediatric neurosurgical patients since improved long-term survival is the rule. Increasing numbers of young adults with hydrocephalus treated during childhood will need appropriate care in the future. Organized transitional care for young individuals only exists in North America and Australia and is limited to a few medical conditions. This chapter will focus on the situation of young neurosurgical patients in transition, the required needs in respect of changing care conditions, and possible strategies for the future.
