Maintaining a well-performing vascular access for hemodialysis is a continuing challenge to the health care team. A well-organized, interdisciplinary approach is required to provide the best access for the longest duration. The native arteriovenous fistula remains the gold standard for vascular access for hemodialysis. Although PTFE grafts are more commonly used, their longevity is limited by the development of venous stenoses. Because of their suboptimal performance, central venous catheters are the least desirable access for hemodialysis.
