Background/Introduction: In spite of representing the third cause of mortality the information about the incidence and prevalence of Heart Failure (HF) in Spain is scarce and predominantly based on hospital data. Purpose(s): To estimate the incidence and prevalence of HF in Catalonia (Spain) in the adult population managed in primary health care (PHC) and the global mortality in this population. Method(s): Retrospective cohort study based on information derived from primary care electronic medical records (EMR) from the SIDIAP database (Information System for the Enhancement of Research in Primary Care). The study period comprised from 1st of January 2010 to 31st of December 2014. Prevalence on 1st of January 2010 and incidence during the period was analyzed. Result(s): A total of 5215912 adult patients managed in PHC on January 1st 2010 according to the SIDIAP database were analyzed. Prevalent HF at baseline was 50230 (0.96%) but in population aged 75 years or more, prevalence raised to 6.47%. Mean age among prevalent HF population was 78.6 (SD 11.10) years and 59% were women. During the study there were 64441 new (incident) cases (2.76/1000 person-year). Mean age among incident cases was 79.0 (SD 10.7) years and women were 55.6%. Most frequent comorbidities related to HF were hypertension, lipid disorders and diabetes. Mortality rates were high, both in prevalent HF patients at baseline (cumulative incidence 41.3%) and in incident patients (cumulative incidence 23.5%) and higher in the first months. Conclusion(s): Prevalence of HF based on the results from our study is slightly lower than in other kind of studies, and focuses especially in the very old population. However incidence was similar to other studies and remained stable from 2010 to 2014. Almost half of the prevalent HF population in January 2010 died during the 5 years of follow up.
