Abstract
Elucidating the syrinx filling mechanism is the holy grail of clinicians and researchers studying this enigmatic disease. This chapter critically appraises the plethora of historical and current theories which have attempted to explain the pathophysiology of syringomyelia. The author also reviews current knowledge on syrinx fluid composition and pressure and hydrodynamic mechanisms and discusses other possible causes of fluid collection including tumours, interstitial fluid, blood-spinal cord barrier disruption, and the role of aquaporins.
