The author describes breast augmentation with cohesive gel implants that he believes are safer, have fewer complications, and lead to better results. In 438 patients there were five cases of unilateral capsular contracture, Becker grade II/III, that were resolved with capsulotomy, and two cases of small superficial hematoma that were drained. Temporary diminution of nipple sensibility was observed in a few patients. There were no infections, seromas, skin necrosis, or hypertrophic scars. A few patients showed lack of pigmentation of the scar, which was resolved with scar revision, laser abrasion, or dermopigmentation. The advantages of tumescent local anesthesia are less bleeding, better recovery, an ambulatory patient, less expensive surgery, and an areola expander effect. © 2009 Springer-Verlag.
