Severe pulmonary hypertension is a serious condition with a poor prognosis. Chronic thromboembolic pulmonary hypertension (CTEPH) results from fibrous organisation of embolised blood clots within the pulmonary arteries causing chronic obstruction and associated vascular remodelling of the microvasculature. Careful assessment of patients with CTEPH by a multidisciplinary team is necessary to decide on the best management plan. Pulmonary endarterectomy surgery is a potentially curative treatment for CTEPH. Operability is dependant on the location of the obstructing lesions and the haemodynamic parameters. In high volume experienced centres, the operative mortality is low and PEA surgery can provide significant long-term symptomatic and prognostic benefits.
