Abstract

Optimizing management of intracranial atherosclerosis is an evolving aspect of neurovascular care in which medical management remains the mainstay of treatment. Endovascular therapy is reserved for patients who fail best medical management with adequate control of associated risk factors and despite dual antiplatelet therapy. Primary submaximal angioplasty is a safe first-line endovascular treatment followed by stenting if necessary.
